When can I sign up for insurance?
Associates have 45 days to enroll in the insurance starting the first day of employment. You will be receiving an email notification on your personal or CITY email with instructions on how to enroll.

When will I receive the insurance information so I can review my options?
You will be receiving an enrollment post card to your home address with a QR code to scan, which will provide you with a copy of our 2023-2024 benefits guide. The benefits guide is also available on the CITY Furniture’s intranet page to view at all times.

When is my insurance effective?
Associates are eligible on the 1st of the month following the completion of a 60 day eligibility period.

Which website do I use to enroll in the insurance plans?
You can enroll into your benefits by logging into the UKG desktop link. Review “How to Enroll in Your Benefits” job aid.

I am not able to log into the “manage my benefits” portion on my UKG profile?
When enrolling or updating your insurance information ensure to use a desktop computer, the app will not let you access your benefit information. Also be sure to use Chrome or Microsoft Edge, both are best compatible with UKG.

Where can I find details about the plans? All the information about our plans can be found in the 2023-2024 Benefits Guide that is posted on the Intranet.

Will I receive a separate ID card for my medical, pharmacy, dental, and vision plans?
You will receive 2 UMR medical ID cards for yourself and each dependent enrolled into your insurance will be listed on the insurance cards. If enrolled into the Grandfathered HRA medical insurance, you will receive a separate HRA debit card with UMR to utilize at the pharmacy. If you have elected the HSA plan, you will also receive a debit card to use towards eligible expenses. A separate Dental and Vision card will be mailed out if you’re a first-time enrollee.

When will I receive my insurance cards?
IInsurance cards are mailed out by the carrier and may take 4 - 6 weeks to receive. If you do not receive your card, you will need to contact the carrier or third-party administrator’s customer service department. The numbers and websites are listed below.

Optum Banking (Health Savings Account)
You can also easily access your balance and most recent transactions from anywhere by utilizing the UMR App or by visiting UMR.com. To take full control of your HSA funds, download the Optum Bank HSA App or visit optumbank.com. The Optum app allows you to take deep dive into the transactions, order replacement debit cards, add your debit card to your Apple or Samsung pay or Google wallet, store receipts, view the eligible expense list, and much more

What do I do if I need to go to the doctor before I receive my insurance card?
You will need to contact UMR for medical and United Concordia for dental at the numbers listed above. The carrier and third-party administrator should be able to confirm benefits with your provider. You can also download your insurance cards on their respective apps.

Who do I contact if I have lost my insurance card?
You will need to contact UMR for medical/pharmacy and United Concordia for dental at the numbers or websites listed above.

How often can I update my Health Savings Account (HSA) contributions?
You are able to update your HSA contributions at any time by creating a life event on your UKG profile – Myself – Benefits - Manage my Benefits. You will need to be mindful of not electing contributions above the IRS limits to avoid any tax implications. HSA 2023 limitations for an individual are $3,850 and $7,750 if enrolled in the family plan. HSA 2024 limitations for an individual are $4,150 and $8,300 if enrolled in the family plan.

How can I see if my doctors are in the United Healthcare Choice Plus network??
Go to www.UMR.com and select Member or Login. Select Find a Provider and search for UnitedHealthcare Choice Plus Network in English or Spanish. You can also search for doctors by specialty, gender, languages spoken, and hospital affiliations.

How do I add my new spouse or child to my insurance coverage?
By logging into your UKG profile, clicking on manage my benefits, you will be able to process a life event. Please keep in mind all life events will require supporting documentation. This must be completed within 30 days of the life event (see below).

Can I enroll now if I didn't enroll during my initial enrollment period?
Associates who do not enroll during their initial enrollment period are ONLY able to enroll during open enrollment (held once a year in September/October), unless they have a qualifying event. Please see below for a list of qualifying events.

  • Marriage, divorce, or legal separation
  • Death of spouse/domestic partner or other dependent
  • Birth or adoption of a child
  • A change in your spouse/domestic partner's employment status
  • A change in your spouse/domestic partner's employment status
  • A change in your dependent's eligibility status due to age, student status, marital status, or employment
  • You or your spouse/domestic partner's change in work hours that affect benefit eligibility
  • Relocation into or outside of your plan's service area
  • Medicaid or CHIP (60 day special enrollment)

Enrollments MUST be completed within 30 days from the qualifying event.

Who would be an eligible dependent?
Associates may elect coverage for a dependent(s) listed below:

  • Your legal spouse
  • A domestic partner
  • Your children who are less than 26 years old; or 26 years old or older, who are incapable of self-sustaining employment by reason of mental or physical handicap and supported primarily by you (proof of ages and dependence must be submitted)

Where can I provide verification documents for my dependents?
You can upload your documents into our secure online enrollment system (UKG) or HR Help. Upload instructions are available on CITY intranet under the UKG tab.

Can I change my life insurance beneficiary at any time?
Yes, Associates are able to change their beneficiary online at any time. Once logged into UKG, click on Myself – Benefits - Manage my Benefits. There you will be able to update your Dependents/Beneficiaries under Profile. Job aid is available on CITY intranet under the UKG tab.

Am I eligible for health insurance if I only work part time?
No, part time associates who work less than 30 hours a week are not eligible for benefits.

Who can help me with questions about the plans?
A Benefits Counselor with US Enrollment Services can help you enroll over the phone or answer any questions you have about the plans. The EONE Benefit Advocate Team (BAT) can also provide answers to your day-to-day benefit questions.

  • Explanation on coverage benefits; help you understand the details of your medical, dental, vision, and life & disability plans
  • Assist with questions regarding your Explanation of Benefits (EOBs) on processed claims
  • Research and assist resolve claim and billing issues
